MEDICATION REFILL POLICY
September 2003
The Hailey Medical Clinic and Sun Valley Family Practice would like to provide our consumers with the following information on medication refills.
We are sorry, but there will be no exceptions!
1. If you believe that you need a refill or change of medication for your most
recent office visit since you feel that you are not improving or have a problem with the medication, please call the office and leave a message on the voice mail for your provider.
2. You must make an appointment to return for any refill on antibiotics, if this is for a new illness.
3. Pain medication and tranquilizers will not be refilled by phone. You Must make an appointment.
Please note that being seen does not guarantee that you will receive pain medication or tranquilizers.
4. Refill requests are processed from 9:00am to 3:00pm Monday thru Friday. Please allow 48 hours for your request to be processed. Medication refills will not be done on the weekends. You must be seen in Saturday Urgent Care if a weekend refill is required.
5. The following policy refers to refills on medications for on-going medical problems. (Hypertension, diabetes, asthma, elevated cholesterol, depression, etc) It does not apply to medications that are used to treat an acute medical problem.
a. It is the patient’s responsibility to make sure they follow up with their
regular clinician for on-going medical problems. The clinician will instruct the patient when to return for regular - interval visits and provide the patient with enough medication to last for that interval. The interval between office visits depends on the stability of the medical problem and medications prescribed.
b. If for some reason the patient runs out of medication for their on going
Medical problem before the next interval visit with their regular provider, the patient may do the following:
· Make an appointment for interval care, if it is due, or call your pharmacy with the refill request and the pharmacy will then contact your provider
· Please allow 48 hours for your request to be processed
· Do not call the clinic to check on your request. This will only delay the process.
· If you cannot wait the 48 hours for a refill, you may be seen in the clinic or walk in on an urgent care basis.
· If a refill is given, it will be for one month to allow a scheduled follow-up with your routine provider.
